This is the self-contained rule in practice: name the numbers, not “baseline.”

“Tolerated treatment well” — what changed? Quantify.
“Stable from baseline” — name the baseline values.
“Resident feels better” — pain score? vital signs? functional level?
“Wound improving” — measurements vs the prior week?
“Compliant with regimen” — adherence percent or pill count?
