The covered skilled-service list (Chapter 8)
Chapter 8 names direct skilled services that, when required, support a covered day. A note that documents one of these — with the reason and the daily need — is on firm ground:

Insertion, sterile irrigation, and replacement of catheters. Care of a decubitus ulcer (Stage 3/4) or a widespread skin disorder; complex/sterile wound care. Care for a recent ostomy in the early post-operative phase. Application of dressings involving prescription medications and aseptic technique. Treatments requiring skilled observation (e.g., initial medical-gas therapy). Rehabilitation nursing procedures and the initial phase of a restorative program. Observation and assessment of an unstable condition; overall management and evaluation of the care plan; teaching and training. |
