The covered skilled-service list (Chapter 8)

Chapter 8 names direct skilled services that, when required, support a covered day. A note that documents one of these — with the reason and the daily need — is on firm ground:

Direct skilled services (representative)

Intravenous or intramuscular injections and IV feeding.

Enteral feeding that meets the coverage thresholds for volume and caloric intake.

Nasopharyngeal and tracheostomy aspiration.

Insertion, sterile irrigation, and replacement of catheters.

Care of a decubitus ulcer (Stage 3/4) or a widespread skin disorder; complex/sterile wound care.

Care for a recent ostomy in the early post-operative phase.

Application of dressings involving prescription medications and aseptic technique.

Treatments requiring skilled observation (e.g., initial medical-gas therapy).

Rehabilitation nursing procedures and the initial phase of a restorative program.

Observation and assessment of an unstable condition; overall management and evaluation of the care plan; teaching and training.
