Categories and format

  • Vital signs — pre/post BP, HR, RR, SpO₂, temperature, pain score.
  • Functional measures — distance ambulated (ft), transfer assist level, Section GG performance, grip strength.
  • Clinical findings — edema grade (1+–4+), wound dimensions (cm × cm × cm), tunneling, drainage, weight.
  • Labs / intake — glucose, INR, weight trend, % meal intake, I&O.
  • Mastery measures — teach-back percent, return-demo accuracy.

Use directional arrows and numeric pairs — they survive every review: RR 26→18; SpO₂ 88→94% on 2 L; BG 312→168 after correction; edema 3+→1+ at 48 h post-diuretic; ambulation 25→75 ft; pain 8/10→3/10 thirty minutes post-PRN.
