Teach-back — for verbal/cognitive content

The learner explains the topic back in their own words. Used for warning signs, when to call, medication purposes, action plans, and dietary thresholds. Document: the specific topic; the method (verbal, handout, video); the audience (resident, family, CNA); the mastery measure as a percent (e.g., “teach-back 80%” = 4 of 5 key points); and a reinforcement plan if under 80%.

Reviewed CHF zones with the resident and daughter (discharge preparation). Reinforced daily-weight technique (same scale, same time, before breakfast) and the red-flag thresholds (>2 lb/day or >5 lb/week). Both verbalized the threshold and the green/yellow/red meaning. Teach-back 100%. Written summary provided. Will reassess next session and add sublingual nitroglycerin use if ordered.
